Access CMS data on certified US skilled nursing facilities. Search by location, filter by star ratings, abuse flags, ownership type, and staffing levels. Compare facilities side-by-side and track changes over time. Returns ratings, nurse hours, turnover rates, inspection scores, and fines.

How to connect: Not in Muse's Connectors list yet, but Muse can still use it. Paste this into Muse: "Use Nursing Home Database to help me. It is a free service with an MCP server at https://mcp.nursinghomedatabase.com/mcp. It does not need an API key. Ask me before you share anything with it." Muse asks before it shares anything with the app's site. Meta does not review apps used this way, so only use ones you trust. We tested this in the Muse app on September 24, 2026: Muse used an app's link directly this way and returned a live answer.

- summarize_facilities: Aggregate numeric CMS ProviderInfo fields (n, mean, min, max; optional median) across certified SNFs. Prefer this over paging search_facilities for national or grouped stats. - get_owner: Get an owner portfolio by web_owner slug, including facility names, locations, and overall stars. Defaults to the latest CMS monthly snapshot; pass filedate from list_file_dates to query a prior month.
- list_file_dates: List CMS monthly snapshot dates available in the database (newest first), plus the current filedate and known unpublished months. Use a returned date as filedate on other tools to retrieve a prior month instead of the latest data.
- list_distinct_values: List distinct CMS ProviderInfo values and counts for a field. Omit field to see the catalog. Use field=ownership before search_facilities to get the 13 ownership types. city requires state.
- get_data_freshness: Get latest source dataset dates and row counts. For prior months, call list_file_dates and pass filedate to other tools.
