Explains why Medicare Part D restricts a drug (prior authorization, step therapy, quantity limit, formulary status) and provides the exception and appeal process with CMS-named appeal levels. Also projects month-by-month out-of-pocket spending against the statutory annual cap for 2026 and 2027.
